How to turn a contact form into an app?

I have a client who owns a restaurant. I created an online feedback form where his manager can answer a few standard questions at closing every night and submit them to him. Is there a relatively simple way to convert a webpage like this into an android app, without knowing how to write code? I know ther are websites like dudamobile that convert a site into a mobile site, but I didnt know if there were any that can convert a site or page into an app. Ive looked but havent found any, thank you for any feedback!

I've searched some more and found a few: appypie.com, and phonegap...both have a monthly fee, but trying them out now. If anyone knows of any other/better solutions, please let me know! Thank you..
