HTC One (M8) — Questions on New Update Issues

There was an update to my phone this morning and I noticed a few issues. I was wondering if anyone knew how to fix or correct them?

1. I use the Slacker Radio App for music. When my phone sits idle and I hit the power button and swipe in to use the features on my phone, I notice the album/artist image appears covering the screen of my phone. Prior to the update, it would just be my wallpaper and a small display of the Slacker app. Is there a way to return this to normal? I tried going through the app, my phone settings, and had no such luck.

2. A new icon is displayed at the top of my screen to left the of ringtone, battery life, time icons, etc. It's a white box with what looks like the letter "N" inside. Anyone know what app this is or how it can be removed?

3. Also despite restarting the phone, recently removed apps still appear and need to be manually removed.
