Samsung Galaxy Note 10.1 — My Samsung Galaxy note 10.1 is Hacked

My Samsung Galaxy note 10.1 got hacked few months ago. Reset it to factory default, still no use.
Few days ago I noticed my Viber account has been hacked.
I uninstall the application. But the rest of the icons did not rearrange automatically.
There's a space where the viber icon is use to be.
Even after resetting it to factory default still there is a space between application icons. And there is another space I noticed on the other panel.
Please help me with my issue. Could it be a Hardware fixed into my device. Or is it a Software?
Even Camera icon looks blurry.

I have never seen any case of android phone hacking
If factory reset is not working than try this

Don't make backup of your phone not of any apps/contact etc. any one can have torjan/malware/virus
Install custom recovery in your Note 10.1
here is the link to TWRP manager download it from playstore
this app requires root access and busybox, you should have rooted phone
open the app and install TWRP recovery as directed by app
go to recovery mode through your TWRP manager
once u get into TWRP recovery go to wipe option in the upper right corner
than go to advanced wipe and select all partitions listed there
this will clear all your mobile data except bootloader your phone will not have OS that time
after this you have to re flash your Note 10.1
for flashing see this video
After you flash your note your phone software will be new and no one can hack it until you give your phone to someone else

This is the only way to get out of hack or just go to samsung service centre and ask them for help
or simply throw your Note 10.1 and buy new one

Call your mobile carrier tell them it has a blurd you have done a factory reset and it did not help if it's been a years or less they will send you a replacement .

I never heard this type of hacking on Android before. I would say stop using your device. See if your carrier can provide replacement in this case else buy a new device for yourself.

The tablet did not get hacked there was some app that was put on the tablet that did that. But I agree stop using the tablet and call your carrier or buy a new one although buying a new one may not be that easy.

When you say you have your phone hacked I suppose you want to say you have Note 10.1 rooted. And the problem you say I will restore your tablet with Kies or smart switch. You can found it on Samsung Web page support area. Or in this link.
