Hello. Here is my app to break phone addiction

Application tracks how much time you spend on your smartphone and how
many times you checked it (unlocked the screen). Also it shows weekly
and monthly charts of phone usage.
With widget you can know your daily usage of smartphone without
opening an app. You can change frequency of widget updates to save
battery energy longer.

It will remind you about your yesterday's smartphone usage every day at
12pm but you can change this time or disable notifications at all.
