Motorola Moto G — Weird lock screen appeared on my Moto G5...have I been hacked?

This lock screen started to appear on my phone:

I'm nervous I've been hacked or something as I didn't set this screen myself. Anyone hear of anything like this?

Hi Sarah,

this has nothing to do with hacking, it's rather an app that's causing this issue. Did you install any new app lately or updated one?

Yes, a QR scanner and poll everywhere... a couple. Thanks. I think I turned it off and I feel relieved it's not a hack.
