Need App Creation Help

Good day to all.

I hope I can get some assistance or guidance on where I can get resources to achieve what I need to improve my office. Any help would be greatly appreciated.

Needs: An app that is capable of receiving input maybe first and last name as a wait list.

Would need to be able to select from 3 or 4 menus/options.

Would need to answer 2 or 3 questions and save on the customers file in order to identify their visit.

Would look something like this.

Office 1
Mike - financial aid - 12:10(check in time)
Joe - customer service - 12:30 (check in time)

Office 2
Jill - job search - 2:30 (check in time)
John - referral - 2:45 (check in time)

Would have to have a user and administrator account to remove/check off list once customers have been assisted.

Just an idea I had and wanted to see if it's even possible.
